Photo Authorization - No Consent

Complete this form if you DO NOT want your child’s photo to be used in any manner (online, social media, video and print advertising).

 

Private School - Affidavit of Intent

Parents are required by statute to file an affidavit of intent with the county school superintendent stating that your child is attending a regularly organized and structured private school. It is not necessary to file an affidavit every year. The affidavit must have the parent/guardian contact information and the student's date of birth. The affidavit form must be notarized.
